did not have anything monitoring what was going on
OpenAI did have monitoring in place. Its own incident report says cybersecurity monitoring alerted the company to unusual activity on July 19, and it had already invested in chain-of-thought monitoring and seen earlier warning signs; the failure was incomplete coverage and escalation, not a total absence of monitoring.
Full reasoning
This claim is contradicted by OpenAI’s own postmortem.
OpenAI says it did detect suspicious activity through monitoring: its technical report states that it detected suspicious internal activity on July 19, and the companion blog says that cybersecurity monitoring alerted the company to unusual Artifactory-credential activity on that date.
The report also says OpenAI had already invested in chain-of-thought monitoring before the incident, but those monitors were not run on the specific evaluations involved here. It further says that an internal team had observed message-board activity and disallowed internet access in May, and that some early warning signs should have triggered an earlier response.
So the factual problem was not that OpenAI had nothing monitoring the environment. The problem, by OpenAI’s own account, was that its monitoring and escalation were insufficiently deployed and acted on.

The extracted official report says: 'OpenAI detected suspicious internal activity on July 19.' It also describes OpenAI’s plan of action around four pillars including 'Monitoring,' and notes that system-level guardrails used in production would have detected the incident as unsafe.

On July 19, our cybersecurity monitoring alerted us to unusual activity involving Artifactory credentials ... before the incident, we had invested substantially in chain-of-thought monitoring ... These monitors did not run on the evaluations in this incident ... an internal team observed an agent engaging in message board activity and instances of disallowed internet access as early as late May.
