- OpenAI’s own 2015 launch post distinguishes its founders from its donors. Jessica Livingston, Peter Thiel, AWS, Infosys, and YC Research were listed as donors, not founders, and Microsoft was not listed in that launch announcement at all.

- This misstates the Dahlöf 1996 study design and sample. Dahlöf 1996 was a five-period crossover trial in which each participant treated separate headache attacks with each test medication, so the study was not limited to 14 people on paracetamol 500 mg.
- This outcome description is incorrect. In the Cochrane review, “pain-free at one hour” means participants who were pain-free at one hour, not those who were pain-free or had only mild pain.

- OpenAI employees did not actually quit en masse after Sam Altman was fired. The widely cited employee letter said they *may choose to resign* unless the board resigned and reinstated him, and Altman later said OpenAI "didn't lose a single employee."

- Jorge Hernández was not 22 when the group first went to San Jose and received the name Los Tigres del Norte. UCLA’s Frontera history places that trip in 1967, and the same source gives Jorge’s birth year as 1954, so he would have been about 13, not 22.
- UCLA’s Frontera history says Los Tigres del Norte’s inaugural recording was the single “De un Rancho a Otro,” not “Juana La traicionera / Por el amor a mis hijos.”
- The table incorrectly marks “De Paisano a Paisano” as a Latin Grammy winner. The Latin Recording Academy lists it as a nomination, while the 2nd Annual winner in Norteño Album was “Quémame Los Ojos/Amigos Del Alma.”

- This is too categorical. In the paper’s own in-context RL experiments, some orthogonal-reward runs did lose monitorability due to semantic drift.
- The paper itself reports orthogonal-reward runs where CoT became unmonitorable, so this “otherwise” claim is too absolute.
